Psa concern post treatment

hi

My partner was diagnosed with stage 4 prostste cancer 4years ago with an initial psa of 48. Biopsies revealed a gleason 4+5 at this time. He has had a course of chemothersphy and radiotheraphy to prostate and a bone metasteses on the iliac crest. He had a good response from chemo to 2 pelvis lymph nodes and para aortic lymph node also at that time as per ct scan post tx. Revised currently on ADT since diagnosis and his psa 3 monthly checks have been 0 06. However in Dec. 25 it was 0.08, Dec.25 0.14 and march 26 0.54. Oncology say happy to wait for a further 3 months to repeat psa as hubby has no current symptoms. I would really appreciate any advise on this please.

It looks like his PSA doubling time is about 1 month so in three months his PSA will be about 4. 

They will need to increase his treatment, maybe more chemotherapy or stronger ADT, such as enzalutamide or arbiterone . The stronger the treatment the more likely he is to have side effects, the sooner they start the treatment the sooner the cancer will learn to find away around it. 

I can see a strong argument for delaying increasing treatment.
