Addition of Apalutamide to Decapeptyl

Hi my husband is currently 16 weeks nearly on Decapeptyl and the PSA done a couple of weeks ago show a decline from 7.1 to .9.

We saw the consultant earlier this week and he said it was a good steady decline.  His Radiotherapy is being scheduled for early July. Because there is a trace in one nodule he was sceptical about a cure. I'm staying positive as I can. It was deemed curable by the team meeting but we know nothing is guaranteed and it may well come back.

He us having 20 sessions RT to the nodes as well.then 2 years on ADT

He is T2 N1 M0.

He is also waiting for Apalutamide to start but the appointment has been delayed from April 2nd to 10th now that cancelled till 28th. He is coping extremely well with the Decapeptyl.  He doesn't seem tired, breaks out in a sweat on his forehead a few times and then takes his jumper off. He has ED which he didn't have before, but because he does want our sex life, to carry on Sildenfil 50g is working very well, at the moment, and he uses the pump most days. We have had a very passionate relationship  for over 51years, something we are both often saying to each other we're very very blessed. As he said over half century of habits is a pretty strong thing. Ive always had a very high libido which helps. !We are coping with this on our own as there appears to be zero help or interest from the hospital. The GP is helpful.

So his desire to keep the PC controlled is much stronger than the fear of side effects.

Is the delay in Apalutamide crucial? The consultant appointment worried me more this time. He said it was fine. We feel it's a good addition to keep it low. He'll hopefully have 2 months prior to RT

Just need a bit of positiveness. Anything negative please don't say as I think this is a stage when I might feel fearful and doubtful.

I think a short delay in starting Apalutamide (by a couple of weeks) is very unlikely to be crucial when he is already on effective ADT with Decapeptyl and has radiotherapy planned. These treatments work over months, not days, and the key disease control is already being achieved — as shown by his strong PSA drop.

Antoinette, In a different situation, but I started apalutamide three months after my first injection.

The injection took the PSA from over 13 to 0.89. The apalutamide and injection combination took the PSA down to less than 0.03.

My first injection was zoladex but my subsequent injections were decapeptyl, the change was because of moving from the private sector to the NHS.

Thanks Chris.

Thank you that's a very helpful comment.  That is what the Radiotherapy consultant said. Yes I think we're doing quite well.

We've always had a fairly healthy lifestyle but I've recently gone back to Slimming World. Their cook books are excellent. Weve also cut out red meat bacon and sausages, and after discovering bread is a superprocessed food, even wholemeal etc, I now make my own cottage loaf about twice a week. I love ❤️ doing it. It's such a pleasure eating it, too, and the house smells lovely.
