Vitaros Cream for non nerve sparing RARP

Hi all,

I was wondering if anyone on here has had success with Vitaros Cream (Alprostadil)?

I read that about 40% of guys who have non-nerve sparing RARP can get an erection via this cream.  I am 2 years post RARP and had never heard of it till I read a post on here.

It has been a nightmare to get.  Being now discharged from the ED clinic, I had to go to my GP.  That resulted in a phone consultation where I don't think my GP understood my situation and I ended up being prescribed a variant of viagra pills.  These do not work for non-nerve sparing ops.  I then had a second phone consultation and the GP refused to prescribe it and instead I have been referred back to the ED clinic.  In my trust, this will take between 3 and 6 months, so is it worth a try or shall I just give up?

I had RT in 2018 and was on HT in 2020. As a result of meeting a lady, at that time getting erections would have been desirable, but HT makes that near impossible. I did get a prescription for Vitaros from my GP, but no high street pharmacy actually stocks the medication, so your GP is not your biggest hurdle here.

I have observed that the NHS doesn't really function anymore, my suspicion is that our MPs can afford to pay for private treatment, so they are not really interested in funding a system for poor people. I decided to do what a rich person would do and get a private prescription from an online pharmacy. It was £65, which I think is better value than waiting 6 months for an NHS ED appointment, which will then be cancelled because of a doctors strike, and then having to wait another 6 months.

Having got the cream and applied it correctly,  to the urethra  at the tip of the penis, I found it to increase blood flow and change the colour of the tip of my penis to bright pink rather than pale pink. It is even possible that the very tip of my penis had an erection, but that isn't much use with a floppy shaft. In short it was useless. Now I'm not saying your experience will be as deflating as mine, so if you can afford it, go online and get some, report back to us. I would love to know if this medication is any use to anybody.

I tried this, you have to squirt it down the end of your knob which I found more traumatic than the injections, oh and it resulted in a limp dick so no improvement really and your partner might not like the active ingredients in her nether regions.

So don't give up! Try the injections! Insist on seeing a GP for a face face or see a urologist privately and get a prescription that way. Might be the best £250 you ever spend.

Hi, Paul.

This conversation is about penile injections and includes how I've dealt with ED after non nerve sparing RARP.

https://community.prostatecanceruk.org/posts/t29845-Hooray-for-Invicorp

I believe your 40% success rate of Vitaros cream, refers to men with general ED issues, not those having non nerve sparing prostate surgery. I suspect the cream's efficacy would be a very, very low percentage for this group.

Vitaros cream is effective in 31 to 40% of people who experience erectile dysfunction. It’s usually recommended for anyone unable to take phosphodiesterase type-5 (PDE-5) inhibitors, likeViagra.

During my research, I discovered that medical professionals use Erectile Hardness Score (EHS) to grade erections.

Grade 1: The penis is larger, but not hard.Grade 2: The penis is hard, but not hard enough for penetration.Grade 3: The penis is hard enough for penetration, but not completely rigid.Grade 4: The penis is completely hard and fully rigid.

I'm nearly 70 years old and research indicates that 75% of all men of my age  beginning to suffer with ED issues, many linked to conditions such as diabetes,  obesity and cardiology problems. Obviously for those who've had their prostates and nerves removed the percentage will be a lot higher.

Prior to surgery I was stating to have ED issues, grade 3 erections, probably caused by pre-existing heart problems, that precluded me from Viagra type drugs.

Invicorp guarantees me a grade 3 erection  within ten minutes, that lasts for at least two hours. Unfortunately, my grade 4's, seem confined to history. 😁

The injections have restored my sense of masculinity. I think who ever invented it deserves the Nobel prize award for medicine. 🙂

Adrian very well put.

Despite being on Decapeptyl and Apalutamide and just finishing week 1 of RT together we are preserving our intimate relationship. It's not funny, not a joke not smutty and certainly not unimportant. He's mentioned it at every appointment.  He's listed what he's doing, the pump we bought ourselves, £296, the Squeezy app for men, the Sildenafil from the GP, and also now the occasional Tadalafil. He's said his wish is to preserve his sexual function as much as possible. Not once has this been mentioned in the follow up letter to his GP. Not once has he been asked if the ED is negatively effecting him. It's the elephants trunk in the room.

I've suggested at his next appointment, after any most important stuff is out of the way he lists all the above again and asks for a comment to be put in the letter. I see it as a kind of insidious negligence
