Sudafed (Pseudoephedrine)

Hi.

Following a radical prostatectomy I was prescribed Sudafed (pseudoephedrine) to prevent emission of urine during sex. Sometimes it works and sometimes it doesn’t.
Does anyone have experience of this? How long before sex should you take it and should it be with, before or after food? And how long does the effect last?

Thanks in advance for your help.

Hi, nmac.

For the past five years, I've been using Sudafed almost daily to ease nasal congestion.

I had non-nerve sparing RARP 3 years ago.

Since the operation, I've had periodic climacturia. It seems to be gradually decreasing but I'm not sure if the Sudafed has had any affect on it.

Since reading your post,  I've done some research on how the drug may affect climacturia. AI suggests although it is sometimes prescribed to ease uncontrolled urination during sex, there is very little scientific evidence suggesting it is beneficial.

Thanks Adrian. I’d assumed that effects of Sufafed were proven - as apparently it is a standard medication for urine escape and climacturia. I’ve found that it works sometimes but it’s unpredictable.
AI also says that only 20%-40% of post-prostatectomy men suffer from this problem although I was told (only after surgery) that everyone gets it.

Hi again, mate.

Since RARP, I have suffered from erectile dysfunction and climactura. I have spent hours searching the Internet for reliable scientific data on both these side effects. I've been dismayed how little there is.

https://urologiacolombiana.com/en/2025-en/climacturia-after-prostatectomy-hidden-struggles-in-sexual-well-being

In this study, we identified a prevalence of climacturia of 30% among patients undergoing radical prostatectomy for prostate cancer in our institution. Similar to our results, Mitchel et al.’s single-center investigation found that the prevalence of climacturia was 44.4% at 3 months and decreased to 36.1% after 24 months. It should be noted that in previous publications, the documented prevalence of climacturia ranges widely from 15.7% to 93%4.

15 to 93% range! How ridiculous is that!

Thanks for the link. The possibility of climacturia was not mentioned or discussed at all when I was deciding on treatment - a serious omission. The focus was all on incontinence and pelvic floor exercises.
On the matter of erectile dysfunction, I have found that using both sildenafil/tadalafil and topical alprostadil works well for me.

Cheers, mate.

I'm not allowed any ED tablets, due to a heart condition, but Invicorp injections, manage to 'raise the dead' 🙂

My urologist said I should take Sudafed if I still had an erection 2 hours after using "dick injections" so maybe it has some secret impact on nether regions none of us are aware of!

Re the climacturia mine has just about stopped 10 years after RARP.
