I'm sorry that you've had to join The Club, but welcome to the forum.
I wouldn't worry about the PNI, your Gleason score is intermediate, your PSA is not too elevated and I presume that you are T2, prostate confined disease. In the grand scheme of things they are a pretty good set of results.
You mention surgery, there may be other options open to you. Here's an excellent video that details various treatment options and their possible side effects.

I was offered surgery or radiotherapy. I prefered RARP. Like you, I had heart issues, and it took a couple of months for a cardiologist to pass me fit for surgery.
