Hi;

My Husband had a psa of 16 (initially diagnosed prostatitis)

Biopsy showed Gleason 3+4 = 7

3 out of 10 cores positive 2 at 3/3 and 1 3/4 found in anterior lobe

Started Prostap in November as he had bowel surgery and surgeon wasn't sure he could get it all (stage t2c/t3a)

1st month on prostap fine, then injection for 3 months - psa dropped from 16 to 0.2

Just had 2nd 3monthly injection and his psa result came back this time as 2.7 - so we are really worried its not working???

Any advice welcome

Husband is 63 no symptoms - biopsy showed no bone/node or seminal vesicle involvement but we are so worried it has jumped t 2.7 whilst on prostap

Ask GP for another test . It could be decimal place in wrong place - ie 0.27

I agree with Ray - check with the GP practice that it is definitely 2.7 rather than 0.27 and then ask for a retest. If it is still up there, ask to see the oncologist early. It could just be that the injection wasn’t given correctly, or was from a rogue batch or even that it hadn’t been stored correctly but I would want to have a conversation with the specialist if it isn’t a one off score.

Would the fact that we are doing calendar month and not 12 weekly make a difference (9 day difference) also he had his psa tested right at the end of the 3 months? Should he get it tested half way through?

Just wondering if there is any other explanation.
Gp surgery only test to 1 decimal point so unfortunately it’s not 0.27.

It’s a strange one as he dropped dramatically mid first injection and this is the start of his 2nd 3month Prostap. Injection into buttock

Total time on Prostap 4 months

Prostate is given as a monthly or 3 monthly dose - it is a different hormone that requires 28 or 84 days. It is worth checking though that he was definitely given the 3 month dose and not the one month version by mistake - it has been known to happen!

Re testing to one decimal place - that only applies to results below 0.1. Once you get higher than that all labs will give results to 2 dp so 0.27 would be a possible reading.

When you ask for the repeat PSA test, ask for the testosterone level to also be checked.

just rang the Gp surgery and it was the 3 monthly prostap and they said the figure of 2.7 was what they got so it looks like it was 2.7.
Seeing oncologist tomorrow and no doubt re-test soon.
Horrible waiting game and hoping it is just a blip as he has only been on prostap a few month and it dropped to his boots then this....

Anyone else had a rise in psa prior to the next prostap injection? we are so worried and bemused how his psa dropped like a stone at to 0.2 8 weeks and 8 weeks later it 2.7 just before next injection.He hasn't been on it very long so we are worried its not working or is it the cancer is just getting angry?

Hi Jenny, did you see the box the PROSTAP came in?

I am on the 3 month injections but have them every 12 weeks as instructed by my oncologist.

Working on the basis Prostap was given correctly.

Possibilty: first injection was aided by pre Prostap injection. Combined they brought Testosterone down to castrate level. Thus its quite possible Prostap by itself cannot do that ( could do with that T test) A change of HT such as Zoladex or an additional drug such as Casodex can overcome that.

In today's PCa world onco has a big box of tricks so a possible set back but you haven't lost yet - not by a long way.

Thanks everyone.
Just out seeing registrar. Retest in 8 weeks and do testosterone levels too. She thinks the Prostap is not lasting in his body for 3 months and that changing to 4 weekly would be better. She said it dropped to 0.2 2 weeks after the monthly injection and it could well be that.
Another waiting game to see if it is down in 8 weeks.
He had a flush whilst we were talking with her so she said it’s definitely doing something.
She told us to try not to worry ( !) but it’s not easy.

just as a matter of interest - what is the policy on giving prostap 3 days early due to a holiday abroad?

I was a Zoladex (the main HT before Prostap came along) guy, both are LHT drugs. A few days before had no issues. Due to your recent Prostap problem it might be prudent to have the following injection 4 weeks afterwards not 4 weeks and 3 days.

My practise nurse set clock for next jab from day of injection. Local support group guys said some nurses took the approach if say Thurs was their day but had it early on say a Tues then we return to a Thursday on next jab. So however many days they were early they were that many days late on next jab. If not too often then no big deal, particularly if been on HT for sometime and no issues. Just in your case not going over for awhile is playing safe?
