Post-prostatectomy: erections returned, now lost with leaks

Hi everyone,


I had a robotic prostatectomy in early November this year with nerve-sparing. The catheter was removed 7 days later, and I’ve been fully continent since then. From day 0, I’ve had tingling, tickling, and pins-and-needles sensations in my penis and perineum.


I started taking 5 mg/day of Cialis on day 17 after surgery (prescribed for 60 days). From that day, I began having nocturnal erections—not very firm, but erections nonetheless. With some mental stimulation, I’ve even been able to get full erections. Of course, no sexual intercourse yet, since the hospital said it was forbidden until day 30.


Until the end of week 4, I had no problems with leaks or incontinence while playing with my partner. But three days ago, things changed.


Now, when I play with my partner, the sphincter sometimes opens involuntarily for a short moment, enough for a little urine to leak, and it can happen repeatedly if we keep going. I can't control it, as I don't realize it is going to be opened until it is. At the same time, the tingling/pins-and-needles sensations have disappeared, and my erections have gone too. I’m not sure why this is happening, and I’m really worried that things are getting worse.


Until now, I’m still fully continent in everyday life, but I’m anxious about sexual function. Has anyone gone through something similar?

Hi Prostateless


You are experiencing arousal climacturia (leakage of urine during sexual excitement and/or at orgasm). I have suffered from it since my prostatectomy 15 years ago. It is, to say the least, a real nuisance and can be embarrassing if you are not able to discuss the issue with your wife/partner.  However it is possible to deal with it and enjoy sex. Both you and your wife/partner have to be imaginative and sympathetic towards each other. We have managed to re-establish our sex life under this new paradigm. Perfectly normal. My journey was similar to yours tbh. In my case these arousal spurts stopped after a while. In the meantime we just used an extra towel. And urine is normally a completely sterile fluid so isn’t ‘ dirty ‘. Good luck

Perfectly normal. My journey was similar to yours tbh. In my case these arousal spurts stopped after a while. In the meantime we just used an extra towel. And urine is normally a completely sterile fluid so isn’t ‘ dirty ‘. Good luck

Hi. Thanks. The strange thing in my case is that at the beginning I didn’t have a single drop until day 25/26. Before that, nothing at all. I don’t know whether this usually stays the same, gets worse, or can improve again.
As for the erections, it’s the same story: I had full erections from day 16 until day 25/26, but after that (about three days ago) they just stopped.


I’ve requested an appointment with the surgeon to understand what the expected prognosis is in my case, because it’s frustrating to feel that everything was going perfectly and then suddenly realize that things can actually get worse.

Thanks, you give me some hope :). How much time in your case lasted this issue?
