I'm sorry that your having prostate problems, but welcome to the forum, mate.
As usual Dave, has given sound advice. An MRI is usually a pretty good indicator of clinically significant prostate cancer but sometimes 'likely' pi-rads 4 results can be due to non-cancerous prostatitis.
Only a biopsy can confirm whether it's cancer and how aggressive it's likely to be. I hope that yours goes well today.
I see that your other post, which is basically the same as this one, was unanswered. That's rare, usually someone responds fairly quickly.
