Survey for partners and family members

The Health Information team at Prostate Cancer UK would like to develop and make our information resources better for partners and family members.

We have created a survey for people who are or have been close to someone who has been diagnosed with prostate cancer. You may be:
· a partner or spouse
· a family member
· a friend.

It should take around 10 minutes to complete and your answers will be completely anonymous.
