Undiagnosed but Symptomatic

Hi Everyone


I am currently experience all symptoms, ED, weak stream, frequent urination, and a pain in right side of my groin.


I’m 52 and recently had DRE which was smooth and normal and have had PSA result of 0.5.

I can’t get a referral with the clear DRE and low PSA but why would I have so many symptoms. BPH or prostatitis would show as enlarged on DRE ? 
Considering going for a private MRI.


Be interested to hear from anyone who can offer any thoughts on my situation ?

It shouldn't matter that your DRE was smooth and your PSA is low - the symptoms are enough for your GP to refer you to urology for a scan on the NHS, but on the normal route not the cancer pathway. You probably don't have prostate cancer so it would be sensible to get a proper diagnosis for the cause of your symptoms. Prostatitis is difficult to diagnose - it wouldn't make your prostate enlarged so wouldn't always show on a scan and it can be highly resistant to antibiotics. Would be useful to ask for a referral to a specialist in prostatitis - you may need prostate massage and / or a big hitter antibiotic.
Also worth noting that your symptoms could be a deep seated urinary tract infection - again, these can be quite difficult to diagnose and treat

I should have mentioned all tests for Urinary infection we’re also normal.

So, how is your GP pursuing your symptoms?


ED should always be investigated. Although it's rarely prostate cancer, it's an early warning sign of many other things, such as diabetes, high blood pressure, atherosclerosis caused by high cholesterol, high BMI, all of which can be secondary to low Testosterone which can also cause ED directly.


You should be being followed for all the symptoms you give, possibly by GP initially, and by referral if they haven't resolved them.


DRE done by GP is not always very reliable, versus DRE done by a urologist who's doing several a day.

As it stands, he isn’t. I was given 3 months of Tamurex to help with frequency.


I believe Testosterone was measured but I haven’t actually had a result. I’ve been more focussed on the DRE and PSA than anything else at this stage.


I will be going ahead with private MRI regardless but they have said they need referral from GP. Crazy that you can’t even pay to have your own body tested without someone else’s permission 🤷🏻‍♂️


UPDATED: Testosterone 21.7 - Normal. Haven't managed to speak to GP yet so no idea if that's good or bad news.

When you see the GP, remind them you still need investigation/resolution for the ED. i would expect them to start with getting full blood counts, plus Testosterone, lipids (cholesterol), HbA1C, blood pressure, etc.


With respect to the MRI, you are looking for an MRI of the prostate in particular and pelvic area in general. Often this is a multi-parametric MRI (mpMRI) with contrast, but some places with newest 3T MRI scanners now do this without contrast, so it might be worth asking the MRI facility exactly what you should ask your GP to refer you for, as GP's don't do the referral for MRI scans on the cancer pathway and so may not know - that's done by Urology.

They did Full Blood Counts already & said they were normal. I don't think they included cholesterol or HbA1C & they didn't check my blood pressure.


I'll raise the questions about MRI. I may also go to a private Urologist at the same hospital as I plan to have my scan.

In which case, get them to order the scan, so it's ordered with the parameters they want.
