Any advice - having seizures

Husband has stage 4 prostate cancer, spread to seminal vesicle, pelvic lymph nodes and one bony met in pelvis.  Gleason 9 (5+4).  Diagnosed April 2022 and on Prostap injections 3 monthly and Enzalutamide tabs 160mgs daily since July 2022.  Also RT to prostate in October 2022.

PSA has gone down really well from 54 at start to latest 0.73.  Last couple of blood tests Alkaline Phosphatase had been slightly raised and oncologists are monitoring this.  

He was unwell on Monday night with nausea and dizziness and then all day on Tuesday the same.  He did vomit but not much and felt better by bedtime Tuesday.  Woke up yesterday and felt fine.  I made a cup of tea and he drank it in bed.  Afterwards sitting on the side of the bed talking to him he suddenly stopped responding, his eyes rolled back in his head, then came back to normal but he still wasn't responding although eyes were open.  Then noticed mouth was twitching a bit and hands also.  Eventually he started responding to me again but seemed to have no idea what had happened then realised he had wet the bed. Called oncology emergency number and spoke to (?) specialist nurse who advised could be due to Enzalutamide but to go to A&E as precaution.

Attended local A&E (different hospital to oncology) who were very good, did bloods, ECG, head CT scan etc and didn't find anything worrying at all.  They discharged him and advised to contact oncology again.

Then this morning before had chance to call oncology he had another 'seizure' (although this time no eyes rolling, twitching or loss of bladder control). Got him onto a chair and he then 'zoned out' again for a minute before coming round properly.  Called oncology and spoke to a different nurse who didn't seem to think Enzalutamide likely culprit as had been on it since July 22.  Advised we need to speak to our own team and she will email them.  Am currently awaiting a call back for further advice.  In the meantime 'zoned out' again at lunchtime, unresponsive etc.  Have been advised to pause Enzalutamide for time being until decision from specialist team.  Also of course no driving etc although he is so washed out he wouldn't want to anyway and I drive so not a problem.

Has anyone else had any similar experiences to this?  Am obviously very worried and a bit at a loss to understand what is going on.  Don't really want him to have to stop Enzalutamide as has been doing to well on it but the potential alternative causes are probably worse.  Need some reassurance please if anyone can give it.

Ask your oncologist about apalutamide instead. If I recall correctly, it has lower stroke risk.

Hi there

Really sorry to hear about your husband's seizures - very frightening for both of you. I am about a year ahead of your husband, diagnosed in Feb 21 but on same treatment plan with Enzalutamide and RT. I have certainly not experienced any seizures but everyone is different and has different reactions. Its a really tough decision when, as you say, the drugs are working so well at suppressing PSA. As Carpe Diem says there are alternatives that have similar effectiveness so worth discussing with Onco about changing to one of them.

I'd agree with Steve,I'm 10 months into enza,side effects for me are very hot sweats at night,loss of body hair,Ed,some bone ache and slight fatigue,I'm slightly younger at 53 next onco appointment 1st of June PSA hovering around the 1 mark,so I'm hoping this will be same at next onco appointment,it's hard to take my body changing but if this is the price to pay for some QOL I'll have to put up with it,I hope the seizures ease and your journey is a long one.

Hi Milly

I myself have been on Enzalutamide for 16 months. Thankfully no major side effects, but PSA has started to rise very slowly. Probably not relevant in your husbands case but I have read in the side effects of Enza. that having alcohol can increase the seizure risk. I have the very occasional drink but it has not effected me. Hope it all gets sorted.

Quick update.  Saw Oncologist today who has ordered a full body MRI scan and to see again in 4 weeks with result.  I don't think she is convinced the Enzalutamide is the culprit but has said not to take it until we have seen her again. Not sure what to wish for as the alternative causes don't sound good either.  In the meantime no driving of course so I am his chauffeur.

Not to put things in your head but I was off my enza for 1 week due to kidney stones being diagnosed in that week my psa went up from 0.9 to 1.4 not a lot I know but up all the same I'm hoping it has come back down on my next blood test next week,I'd have a blood test in a week or so if your able just to keep an eye on the psa.

Thanks for reply Phil.  Well we saw consultant with results of MRI and blood tests and all good thankfully.  Nothing untoward found on MRI (other that what we already knew about of course) and PSA is still coming down despite no Enzalutamide for several weeks. Cause of seizure remains a mystery.  Husband was keen to go back on the Enza despite the risk of further seizures so that is what is happening.  Meanwhile he has had to surrender his driving license for at least 6 months (and longer of course if he has another fit).  Just hoping it was a one-off unexplained event and doesn't happen again - the dose has been reduced so that might help - keeping fingers crossed!
