Soreness and pain in perineum/penis

Hi gents (and supporting ladies), after a transperineal biopsy I started to feel a hot and persistent pain (like a red hot ball bearing) behind my scrotum which radiates to the left testicle and also makes the penis shaft and penis tip super painful. This got worse after focal cryotherapy in March 2023 for my grade 2 cancer. The best news is the cancer may have gone cos my PSA is back down after the cryotherapy, but the pain (which docs describe as prostatitis or LUTS or CPPS) is still there and getting worse. Some days it is so bad it feels like I've been kicked by a horse in the groin and I can't run or play any sport. Any tips on what is causing it and/or how to treat it? I've tried all the usual conventional medicines and the docs are giving up on me. Thanks!

Have you been checked for a UTI or Bacterial Prostatitis?


I'm guessing they will have run full bloods before doing the cryotherpy, so hopefully they would have seen any significant infection markers?

Thanks, good question and there is some doubt in my mind about this: I've had lots of routine urine samples taken and they always come back within 30 minutes or so saying my urine is clear, but I don't think I've ever had proper lab tests done.


Likewise with blood: lots of samples taken but I've no idea whether they were looking specifically for infection.


I've never had a fever.


I've had two courses of antibiotics (doxycycline and trimethoprim). Neither had any impact.


Their very first diagnosis for my symptoms was epididymitis.


Do you have any suggestions as to what to do next, or for any treatment options?

Urine tests should not be available the same day if testing for infection - they are cultured in the lab for at least 24 hours.

Good point, thanks, I will check my records to see if that ever happened. I don't think so. But if I had an infection wouldn't I have had a raised temperature or something?

All records show no growth so I assume that means no bacterial stuff spotted. Next stop the andrology specialist - a mere 18-month waiting list 😞
