Psoriasis and Prostate Cancer

Diagnosed with PCa in August 2021. RARP in Nov 2021, Gleason 9 with evidence of metastases on one side. RT plus ADT for 6 months, but PSA started rising in late 2023. Now on Firmagon (ADT) plus Darolutamide for the last 2 months; but psoriasis is really flaring badly and topical treatment is not helping. I believe the ADT drugs may be the cause of the flare. Planning on starting systemic treatment (either Apremilast or Sotyktu) for plaque psoriasis soon, but none of the specialists know whether the combination of prostate cancer drugs and psoriasis drugs will have any serious side effects.

Anyone in the community experiencing psoriasis along with their PCa? I would like to know your experiences, and happy to share ideas.

It's not something I've noticed as a psoriasis sufferer myself but I haven't had the specific drugs you mention. I may, at some point, require extra treatment so I will monitor my psoriasis and this thread.

Not relevant but I had/have a patch on my left hip (amongst other areas). When I had my radiotherapy some of which seemed targeted at that area, it almost disappeared. Came back eventually but never as bad as before.

Hi just to say that inositol is good for psoriasis some men take it for prostate cancer too its very safe no side effects you might want to look into it

Hi yes I know inositol helps people on lithium, I will try to find a link to it helping with prostate cancer 

The reason inositol helps  in both cases is  because it is  estrogenic.  Lithium depletes estrogen so I am guessing you are getting psoriasis because of the lack  of estrogen due to ADT although I am not a medical person. Inositol is safe you know if you are taking too much bc it will cause diarrhea it also makes your hair grow so it counters male pattern baldness (not saying you need it for that lol)

If you type in inositol and prostate cancer it will come up with many sources here is a link to guys on the US inspire site discussing inositol  some of them are taking really high doses  I think the benefits are due to it being estrogenic I think men should be on estrogen patches rather than ADT but that's another subject

You need to discuss that with your oncologist before you even think of trying it.

You could ask about swapping from Degarelix to Stilboestrol which is a synthetic estrogen, it is considered quite an old-fashioned treatment now but can be very effective for men who cannot have the usual ADTs or when other ADTs have failed. Unfortunately, I don't think you would be able to swap until the Darolutimide stops working.

Hi Ross yes there arent many mentions of the cause of psoriasis I guess genes play a part I get eczema given the right circumstances. I still maintain its low estrogen at least for women which is a bit unusual in that most autoimmune disorders are due to too much estrogen and men dont get autoimmune disorders as much as women presumably due to testosterone - so is it the lack of testosterone or estrogen causing your psoriasis ? either way the beauty of inositol is it isn't anything radical that can harm you but as Lyn says you should run it by your health team first its also good for insomnia

Thanks Lizzo37. So this D-chiro-inositol? Also called Vitamin B8 (although it is not a vitamin).

I have only found articles on the use of inositol for patients with psoriasis caused by lithium (taken for bipolar disorder). I cannot find any studies or informed commentary on the use of inositol for "normal" plaque psoriasis.

Any idea just why it helps psoriasis?

Yes, I have found a few studies regarding the use of inositol for prostate cancer, but I am more interested in the claims regarding inositol for psoriasis reduction and prevention.

I know ADT for prostate cancer reduces testosterone production, and estrogen is a metabolite of testosterone, so it makes sense that ADT also causes reduced estrogen levels. I have found some articles that claim low levels of either testosterone OR estrogen can cause psoriasis flares. I wonder if taking inositol along with the ADT I am undergoing can help reduce my psoriasis problem?

As I understand it, Darolutamide is an additional drug to support ADT by limiting the ability of any remaining testosterone to adhere to the cancer. My cancer is still in the hormone-sensitive stage and the darolutamide extends time duration of this stage of the cancer and therefore overall survival times.

My oncologist has stated that the Firmagon (Degarelix) is holding the PSA at zero for now and I should try drugs suggested by the dermatologist to reduce my psoriasis. If negative interactions occur I can drop the darolutamide to see if that is the cause of the interaction.
