Prostap 3

I’m currently 18 months into a 3 year programme of Prostap 3. Main issues are swollen stomach and libido and erectile difficulty.

I have a telephone appointment with the consultant in July and intend to ask about the need for 3 years treatment when a lot of opinion says that 18 months is just as effective. I’m expecting some kickback!
My dream is that I stop Prostap and suddenly I can get into my trousers then get the urge to whip them off!😂

Any thoughts, advice etc. greatly appreciated

Looking back, you were T3N0M0 G4+5, which is a high risk diagnosis. It would be interesting to know what your PSA is now, but your PSA wasn't very high to start with, so that might not be a useful indication.

I think the thing to do would be to ask your oncologist what your chance of a cure is with 18 months HT versus 36 month HT.

What is important is to be having regular erections to preserve function. If you can't do that naturally, you should be doing it using Tadalafil and/or a pump, whatever works for you.

I'm sorry to tell you that from personal experience, stopping the HT doesn't magically make the weight fall off. Libido comes back when your Testosterone returns, which happens quite suddenly somewhere between 6-15 months after the last injection runs out.

Thanks Andy that’s really useful. Tbh I’ve stopped using sildenafil simply because I have no urge but you’re right I need to get back on the horse. It’s been a little disappointing that I haven’t been referred to a specialist clinic or such for the ED issue but it’s probably something else to ask. I’m looking forward to an interesting conversation! 18 months further HT plus up to 15 months waiting for testosterone reappearance is a long time! Which is why I’d like the early conclusion!
Thanks again

The postcode lottery makes me so cross! Our ED nurse supports men post-op but also men on HT and our CCG approves vacuum pumps for men on curative RT/HT (but not men on lifelong HT). Ask your consultant whether there is an ED or andrology service in your area and whether it is the consultant or your GP that should make the referral

Thanks Lyn I’m definitely going to mention it. Unfortunately the telephone consultation sometimes feels like a formality. I don’t even know if they want me to do any blood tests prior to the call? Not sure how he can assess progress or current situation without doing so tbh!

If your appointment is more than 3 months since your last PSA test, I would try to get a PSA test beforehand so you have the result at the consultation. The PSA level may factor in to the recommendation to continue on HT or not. You could ask your CNS or GP for a blood test form and get it done in plenty of time for the appointment (like 2 weeks before).

You've got a fair few things to ask at the consultation. Write the questions down now (and any more as you think of them), and then go through them on the call. If you would prefer a face-to-face consultation, ask. Most hospitals are doing these now, but they are often not the default unless you ask.

Good idea Andy. I’m going to ring our Urology dept on Monday and discuss these issues.
